Please keep all packing material and documentation in the event that your equipment
has to be serviced or returned.
Before returning any product, you must obtain a Return Merchandise Authorization
(RMA) number. NO returns, of any type, will be accepted without an RMA number.
Please have the following information on hand when calling for an RMA number:
customer name, invoice number, serial number and the nature of the problem. To
obtain an RMA number and return instructions, please contact our Returns Department.
All defective products will be accepted by AmputeeSupplies.com for credit, exchange or
repair, at AmputeeSupplies.com's discretion within 30 days from the invoice date. After
30 days, the manufacturer's warranty applies.
Some non-defective products may be returned for credit or exchange. Those returns
authorized by AmputeeSupplies.com may be subject to a restocking fee of between
15% and 25%, and must be completed within 30 days of the date of invoice. Orders
received after 30 days may be refunded as store credit for future purchases only.
All products to be returned must be returned within 30 days from the invoice date,
100% complete, in resalable condition, and must include original packing material,
manuals, blank warranty cards and other accessories provided by the manufacturer.
AmputeeSupplies.com reserves the right to authorize product returns beyond 30 days
from the invoice date. If the product is accepted after 30 days, credit will be issued
toward FUTURE PURCHASES ONLY.
Certain products may not be returned due to hygiene issues. In addition, we
cannot accept certain products, ie suspension liners, sleeves, Skin Repair and Skin
Care products that have been removed from their original packaging.
Damaged Goods
Damaged or short shipments should be reported within 5 days of receipt.
If a shipment arrives damaged or if the box appears damaged...
Note damage on the delivery bill and have the driver sign it, acknowledging the
damage. Contact AmputeeSupplies.com Returns Department within 5 days of receipt to
start processing an insurance claim. An order will be entered at that time to replace the
merchandise. Save the container and merchandise for inspection.
