Need to return your prosthetic supplies?  Please read the following Instructions
and General Return Policy returning your order.

Step 1:
Before returning anything, you must first obtain a Returned Merchandise Authorization
(RMA) number.

An RMA number can be obtained by completing the
RMA Number Request Online Form
below.
It is important that the RMA number is clearly written on the outside of the box.
Packages received without an RMA number will be refused.  

Please note that an RMA number does not guarantee final disposition and all
returns are subject to inspection.  
RMA numbers expire 2 weeks from email delivery from our Returns Dept.
Please check your email's "Spam", "Bulk" or "Junk" folders before sending a second
RMA number request.

Orders Arrived Damaged
If your order arrives visibly damaged please refuse the shipment. This will allow our
Team to file a claim easily with the carrier and simultaneously facilitate your
replacement.

Upon acceptance of the shipment from our warehouse, the shipping carrier assumes the
responsibility for safe delivery.  If you receive a shipment with damaged items
(including concealed damage) please follow these steps:

1.  Do Not return the damaged goods to us without first obtaining an RMA number and
having the packages inspected by the shipper or its agent.
2.  Retain all shipping cartons and packing materials for inspection by the carrier.
3.  If received via UPS or FedEx: complete the RMA Number Request Online Form within
24 hours of receiving the shipment to start a claim with the shipper. An inspection may
be necessary to complete the claim.
4. In cases of concealed damage, pictures showing the damage upon arrival could be of
great help in getting reimbursed from the shipping carrier.

Defective Orders
If you received a faulty item and need to exchange it for the same item, complete the
RMA Number Request Online Form
below to request an RMA number to initiate the
replacement process.  Please describe in detail what you consider defective in the
"Comments" field. We'll pay the return shipping costs if the return is a result of our
error and/or the item is defective.

Exchange Your Order
If you would like to exchange an item for a different one, please return the original
item by completing the
RMA Number Request Online Form below (you will be refunded
your original purchase price less initial shipping and any applicable restocking fees
within 30 business days of mailing your return.) Then place another order for the item
you wish to purchase. You do not have to wait for us to receive the original item before
placing the new order.

Wrong or Missing Items
If you received the wrong item or an item you didn't order, please complete the RMA
Number Request Online Form
below to initiate a return.

If you're missing an item, check the packing slip that was included with your shipment.
In order to fill your order quickly with items already in stock at different warehouses,
we may have split your order into multiple shipments, which we would indicate on the
packing slip.

If this is the case, you will not be charged any additional shipping costs beyond those
you originally authorized.

Step 2:
You may return most new, unopened and non-defective items sold by AmputeeSupplies.
com within 30 days of delivery for a full refund less initial shipping charges.

product lines (such as Skin Care, Below Knee Sleeves, Prosthetic Socks, Prosthetic
Shrinkers, etc.).
Please also read the list below of Non-Returnable Items.

If an item was sold as a set, individual elements of that set cannot be returned for a
refund. (Please return the entire set if you are requesting an eligible refund or
replacement.)

All supplies must be in original factory carton or plastic bag (non-torn), including
all packaging materials, inserts and manuals, warranty cards (not filled-out) and
all accessories.  Please do not tape or write anything on the item or the original
manufacturer's carton.  In some cases, we may charge you for the replacement
of damaged, altered, missing, written-on or taped-on contents or cartons.  We
reserve the right to refuse any such returns.

If you paid for the purchase with a credit card or PayPal your account will be credited
back the amount of the purchase, less initial shipping charges and any applicable
restocking fees, once the product has been received and is in compliance with our
General Returns Policy and Terms of Use.  

Please note: Credit for returned items will be issued to the purchaser's original
payment method. Those returns authorized by AmputeeSupplies.com may be subject to
a partial payment (restocking fee), and must be completed within 30 days of delivery.  
Orders received after 30 days of delivery may be refunded as store credit for future
purchases and will be subject to restocking fees.

Packing and Sending Your Return
You can pack and send your return to us in just a two simple steps.

1. Pack the items and packing slip securely in a box. You can use the box the items
arrived in or another box, if you prefer. If you do not have the packing slip, please at
least send a print out of the email containing your RMA number.
2. Once your Return is
appropriately packed take your package to a post office or
shipping center.

Step 3:
On most product returns, you are responsible for arranging shipment of the product
back to the
appropriate AmputeeSupplies.com returns address provided in a
response email from our Returns Department.
 DO NOT mail it to the address on
your order's packaging. You may use UPS, FedEx, or any other ground parcel carrier of
your choice.

If you are returning an item that is defective or shipped in error by AmputeeSupplies.
com, we apologize for your inconvenience. We'll pay the return shipping costs if the
return is a result of our error and/or the item is defective. We do not reimburse or
refund shipping charges if a third-party shipper is used such as Mail Boxes Etc., UPS
Store, Postal Annex, etc.
Note: A signed proof of delivery will be required on all lost
package claims.
 For any other return, you are responsible for the initial delivery costs
and the return shipping expense.

Returns Policy Overview

If you have an Amputee Supplies, Inc. private label product or other product that is
eligible under a warranty issued by Amputee Supplies Inc., you can always choose to
exercise your rights under that warranty rather than going through our General
Returns Policy.

Items that are Not Returnable
AmputeeSupplies.com does not accept returns of the following supply:
Opened Skin Care and Skin Repair products such as, but not limited to Lotions,
Balms, Antiperspirants, etc.
Opened Prosthetic Shrinkers
Any Skin Care and Skin Repair product more than 60 days after delivery
Any product regardless of unopened, more than 90 days after delivery
Any product missing the serial number or UPC
Gift cards

Calculating Your Refund
The following conditions and exceptions apply, as do those listed on the Product Specific Policies pages.

Restocking Fees:
Any opened item in resalable and original condition within 30 days of delivery:
15% of item's price.
Any opened item in resalable and original condition that is returned more than
30 days after delivery: 40% of item's price.
Any item that has obvious signs of use: 60% of items price
Any items that have been opened and are NOT in their respective original factory
carton or plastic bag: up to 60% of item's price.
Any item that is not in its original condition, is damaged, worn, missing parts or
partially used for reasons not due to our error: up to 80% of items price.

Quantity Discounts
You may return items for which you received a quantity (either by volume or dollar
amount) discount in accordance with our general return policy.

Note: If by returning an item your original purchase is no longer eligible for the
quantity discount, the discount received will be subtracted from the purchase price of
the item being returned. You will be refunded the difference.

Refund Processing Times
For items directed to our Returns Center, expect your refund within 4 weeks of giving
your package to the return shipper (though in many cases you'll receive the refund
sooner).

This time period includes:
5 to 10 business days for us to receive your return from the shipper
7 to 10 business days for us to process your return
The time it takes your bank to process our refund request (see below for specific
guidelines)

We'll notify you via e-mail of your refund once we've received and processed the
returned item.

If the return and refund do not appear on your method of payment within 4 weeks
from the day you sent us your return, please contact us for help.

How Refunds are Issued
Credit Card - 3-5 business days after return is processed
PayPal - 1 business day after return is processed
Credit card and gift card - Credit card first, then gift card balance, 3-5 business days
